About the role
We are seeking an experienced and committed Adult Social Worker to join a Learning Disabilities team. You will support adults with learning disabilities to live safely, independently and meaningfully, while promoting choice, control and inclusion in all aspects of their lives.
Working within a strengths-based and person-centred framework, you will complete Care Act assessments, reviews and support planning, ensuring that individual outcomes remain at the heart of your practice. You will work collaboratively with adults, families, advocates, providers and colleagues across health and social care to identify appropriate support and manage changing needs.
Your responsibilities will include safeguarding adults, undertaking assessments of risk, contributing to best-interest decisions and arranging suitable packages of care. You will also monitor the quality and effectiveness of commissioned services, respond to concerns, maintain accurate case records and ensure that all work complies with relevant legislation, policies and professional standards.
The successful candidate will hold a recognised Social Work qualification and current registration with Social Work England. You will have sound knowledge of the Care Act 2014, Mental Capacity Act 2005, safeguarding procedures and the principles of positive risk-taking. Experience of working with adults with learning disabilities, complex needs or associated health conditions is highly desirable.
You will be confident managing a varied caseload, prioritising competing demands and making well-reasoned professional decisions. Excellent communication, assessment and report-writing skills are essential, together with the ability to build effective relationships and work constructively as part of a multi-disciplinary team. A commitment to equality, diversity and strengths-based practice is expected.
This role offers the opportunity to make a meaningful difference to people’s lives within a supportive professional environment. You will be expected to participate in supervision, ongoing learning and service development, helping to improve outcomes and promote high standards of adult social care.
