Contribute to the assessment, mobilisation and ongoing management of care packages, working closely with the Operations Director / Service Lead to ensure appropriate and safe service delivery.
Undertake and review behaviour support plans and safeguarding documentation, contributing to escalation planning and incident reviews where required.
Act as a key point of contact for Case Managers, MDT members, NHS professionals, Commissioners and families, attending MDT meetings and safeguarding reviews as appropriate.
Monitor and evaluate the quality and outcomes of care packages using SweetTree’s Outcome Measuring Tool.
Promote the use of SweetTree technology, including SweetTree Connect and SOMA, supporting the team to improve communication, monitoring and client outcomes.
Actively participate in the recruitment of support workers, ensuring the right skills and experience are matched to individual client needs.
Conduct regular support worker supervisions, appraisals and performance reviews, encouraging reflective practice and professional development.
Support and deliver training for new and existing support workers, including relevant competency assessments and sign-offs.

Participate in the Emergency Out of Hours / On-call rota.
Work collaboratively with all members of the Brain Injury team to promote a positive and cohesive working environment.
Be responsible for taking Brain Injury/Neuro referrals and enquiries, logging information, providing advice and following up as appropriate.
