Practice Nurse

​We are looking for a qualified and experienced Practice Nurse to support a GP practice in the Peterborough area. This opportunity involves delivering routine practice nursing services alongside the management and monitoring of patients with long-term health conditions. The practice uses SystmOne, and the successful candidate will be confident providing patient-centred care within a busy primary care environment.

With sessions available on Thursdays or Fridays, this position is suitable for an experienced Practice Nurse looking for flexible clinical work.

Responsibilities

• Deliver routine practice nursing care in accordance with established clinical guidelines.

• Support the assessment, monitoring and ongoing management of patients with long-term conditions.

• Conduct appropriate patient health assessments and routine clinical observations.

• Provide health education and advice to support patients in managing their conditions.

• Maintain accurate and up-to-date patient records using SystmOne.

• Identify changes in patient health and escalate clinical concerns when appropriate.

• Work collaboratively with GPs and the wider primary care team to support continuity of care.

• Follow relevant infection prevention, safeguarding and patient confidentiality procedures.

Requirements

• Relevant nursing qualifications and current NMC registration.

• Previous experience working as a Practice Nurse within a UK GP practice or primary care setting.

• Experience supporting patients with long-term conditions.

• Proficiency in using SystmOne for clinical documentation and patient management.

• Good understanding of routine practice nursing procedures and primary care protocols.

• Strong clinical assessment, communication and organisational skills.

• Ability to work independently and collaboratively within a multidisciplinary team.

• Availability to work on Thursdays or Fridays.
