Complex Care Coordinator
Location: Heywood, Greater Manchester
Salary: From £28,000 per annum, plus bonus and on-call allowance
Hours: Full-time, Monday to Friday, plus shared on-call
Requirements: Previous Care Coordinator experience, full UK driving licence and access to your own vehicle
About the role
We are recruiting an experienced Complex Care Coordinator in Heywood, Greater Manchester, to support the delivery of high-quality, person-centred care in our clients’ own homes.
This is an opportunity to make a real difference to people living with complex healthcare needs, ensuring they receive the right care and support from a consistent, well-matched team of Healthcare Assistants (HCAs) and nurses.
You’ll take responsibility for coordinating care packages, managing staff rotas, responding to changing client needs and building strong relationships with clients, families and healthcare professionals. You’ll also represent the people we support at multi-agency meetings, helping to ensure their needs remain at the centre of care decisions.
If you have experience in care coordination, enjoy a fast-paced role and can make confident decisions while balancing the needs of clients and staff, we’d love to hear from you.
Key responsibilities
- Care coordination: Act as the first point of contact for clients, families and healthcare professionals.
- Rota management and scheduling: Coordinate shifts and match Healthcare Assistants and nurses to care packages, maintaining safe and consistent staffing.
- Client care and support: Monitor care delivery to ensure it meets individual care plans, client needs and quality standards.
- Family and professional relationships: Build trusted relationships with clients and their families, keeping communication clear and supportive.
- Multi-agency working: Attend Integrated Care Board (ICB) meetings and work with other professionals to support the needs of clients.
- On-call support: Participate in the shared out-of-hours on-call rota, helping manage staffing issues and changes to care delivery. This is a coordination role, not a hands-on care position.
What we’re looking for
To be successful in this Care Coordinator role, you’ll have:
- Previous experience working as a Care Coordinator, ideally within complex care or community healthcare.
- Strong organisational skills and experience managing staff rotas and care schedules.
- The ability to make calm, confident decisions in a changing environment.
- Excellent communication skills, with the ability to adapt your approach to clients, families and professionals.
- An understanding of person-centred care, care planning and the importance of maintaining quality standards.
- Confident negotiation and influencing skills, with the ability to build effective working relationships.
- A full UK driving licence and access to your own vehicle, as travel across the North West region is required.
What we offer
In return for your skills and experience, you’ll receive:
- From £28,000 per annum
- Bonus scheme and on-call allowance.
- 25 days’ annual leave, plus Bank Holidays.
- Your birthday off.
- Career progression opportunities.
- Loyalty rewards.
- Refer-a-friend scheme.
- Paid DBS check.
- Workplace pension.
- A supportive working environment with a focus on work-life balance.
